Mildred is one of our most rewarding success stories. When we were contacted by her niece regarding Mildred’s care, she was in a rehabilitation facility after breaking her hip. She was unable to do most activities of daily living and all instrumental activities of daily living. The discharge planner at the rehabilitation facility had recommended nursing home placement. Her niece, her closest source of care, travels frequently, lives at least 45 minutes away, and was very upset at the thought of placing Mildred in a nursing home. During her visits to various nursing home facilities, Mildred’s niece was referred to Distinctive Care by an admissions coordinator.
Sophia at Distinctive Care conducted a typical assessment which included cognitive testing, a health review, and a 130 point safety assessment of her home. An individualized care plan was developed keeping Mildred’s safety and independence in mind.
The care plan included recommendations to Mildred’s home and caregiver assistance in the home. Prescribed changes were made in the bathroom and bedroom which enabled Mildred to move about her home more easily using her rolling walker. A live-in certified home health aide was found through a partner home health agency to assist Mildred with her activities of daily living, cooking and light housekeeping. Additionally she has become a true companion to Mildred, spending time singing songs and playing her favorite game of Chinese Checkers.
Not only was Mildred able to be brought home, but her two cats Buffy and Rusty, who mean the world to her, were able to continue to live with her. Mildred continues to entertain her friends in her home on a regular basis. Her niece is able to continue her activities and travel without worry knowing Distinctive Care is managing Mildred’s medications and checking- in on her weekly. Distinctive Care schedules and accompanies Mildred to all medical appointments, ensuring that recommendations and referrals are followed through, and her niece is advised of all outcomes.
On two occasions Mildred had to go to the emergency room and was accompanied by staff along with her Emergency Binder prepared by Distinctive Care. Because this binder includes all current medications, past medical history, emergency contacts and legal documents, she was able to receive prompt and appropriate care upon admittance. Our staff member stayed with her until she had a room and was feeling safe and comfortable.
Upon interviewing Mildred for her success story, her further comments regarding her care by Sophia were: “An angel. My Sophia is wonderful. She’ll do anything to help me. I really do depend on her. I couldn’t be without her. We have good times together. She’s down to earth and does the best she can for me.”
Sophia continues to enjoy weekly visits with Mildred. She brings her favorite pizza, which they share for lunch while catching up on Mildred’s week.
