- You notice that Mom or Dad is acting different; the home has become cluttered, they can't pay their bills, balance their checkbook, prepare meals or get lost when driving in a familiar area, etc.
Ways we can help families adjust to, and cope with changes during the aging process:
- Assess the individual to determine strengths and needs, develop an individualized plan of care with personalized services to meet the clients' needs and wants, offer assistance in implementing the plan, and provide assistance with ongoing care.
- Research appropriate long-term care living arrangements and assist with the move-in process and adjustment period.
- Provide referrals to other professionals, including geriatric medical professionals, elder care attorneys, and financial planners.
- Screen, arrange and manage assistance in the home.
- Act as a liaison to families at a distance, ensuring peace of mind, knowing that a licensed professional is monitoring, and managing your loved one's needs.
- Prevent unnecessary hospitalizations.
- Decrease family members' time away from work, and allow the family to spend pleasurable time together.
- Provide Pet Therapy.
- Arrange for driving assessments.
