plan, and provide assistance with ongoing care.

with the move-in process and adjustment period.
- Provide referrals to other professionals, including geriatric
medical professionals, elder care attorneys, and financial planners.
- Screen, arrange and manage assistance in the home.
- Act as a liaison to families at a distance, ensuring peace of
mind, knowing that a licensed professional is monitoring, and managing
your loved one's needs.
- Provide counseling to clients and family members to assist in
the transition to a new stage in life.
- Monitor chronic illnesses in order to decrease hospitalizations
and improve quality of life.
- Decrease family members' time away from work, and allow the family
to spend pleasurable time together.
- Provide Pet Therapy.
- Arrange for driving assessments.
