Distinctive Care is a geriatric care management practice founded on the principle that older adults are entitled to maintain an independent, autonomous and dignified lifestyle.

With a staff of nurses and a trained therapy dog, we strive to provide peace of mind for the older adult and the family that cares for them.

Our services include assessments, evaluations and individualized care plans. We are able to arrange for in-home care and management, medication management and care coordination. Our vast knowledge of costs, quality and availability of services enables us to make appropriate referrals. Additionally, our staff is trained to handle crisis situations that may arise.
